I think the only possible link to polyamory that I could find in the questionnaire was question #5 when it asked what clubs/organizations you belong to and one of the check options was: Lifestyles/NASCA/or other similar group.
Then, #6 asks you for the specific name or nature of the group. I know that Lifestyles is a "play couples" group (read swingers). I am not even trying to tip into the swingers and polyamorists are similar, but polyamory could be included in that subgroup and defined by question #6.
Big shout out to my swinger friends who broadened my horizons enough to know what Lifestyles are/is. Woo hoo!
